To function as a highly skilled and experienced practitioner within an established and specialist registered service in Medway. Shared Lives provides a flexible, individual and diverse service across Medway for adults over the age of 18 with a learning disability.

The role will involve supporting the team manager in promoting and maintaining standards and supervising staff members and students with a range of experience and qualifications. To carry a caseload of complex cases and support the team in developing and supporting long-term placements, short breaks, kinship support, day support and parent support.

You will have significant experience in working with people with leaning disabilities and/or complex long term conditions, a flexible attitude and a willingness to join the out of hours on call service operated by the service. A recognized social work qualification (DipSW, CQSW or equivalent) is required with excellent interpersonal skills and a commitment to provide person centered and high quality individualized services.
